For attaching the power transistor to the heatsink, should I use the middle hole on the heatsink, which lets the transistor leads go fully through the PCB, or the top hole, which means the leads just touch the PCB, and perhaps solder from the top?

I typically use the middle hole.

Soldering just on the top would also work, I suppose, but might be less durable.
