How do I connect the AES/EBU input to the Single S/PDIF Level Converter? I see only two points for the input (IN+ and IN-). Where does the GND for the AES/EBU get connected to the board? Thanks.

Just leave the GND unconnected. It is not needed for differential signals such as AES/EBU.
