I’ve wired my set-up very close to the dual mono diagram but, on both outputs (XLR’s to my amp I’m not using volumite) I’ve hooked up pin 1 to the GND output on the side of the IVY, does it have to be connected exactly as the diagram in regards to ground? For input I’m using a single toslink module powered from one of the placids, I’ve connected up both GND and D1 to both buffs, lock works on both DACS. Maybe reverse the phase on one channel?

Thanks Brian, I must confess I didn’t change the DIP switches as per the manual for dual mono configuration. It’s working a treat now!

I’ve been listening for a while with 16/44.1 and it sounds great. If I change over to 24/96 or any other higher rate I get a crackling noise in the right channel. I’m not using a volumite, straight output to my amp. Look like you are using SPDIF.
If you are then you don't want to feed the same signal directly to both DACs. You will have a double termination. This may not be your issue but its worth looking at anyway.
Instead of what you are doing there (going direct into SPDIF both DACs) feed the SPDIF into DAC #1 normally with the SPDIF switch closed as usual. Then wire from GND and D1 on DAC #1 to GND and D1 on DAC#2 and keep the SPDIF switch *open* on DAC #2.
Now you have a single termination for the SPDIF and you will be feeding the second DAC with TTL signal from the comparator of the first. As a bonus they will be in synch as one comparator will drive both.
Give that a shot.
