What was the issue? The initial transition of opamp from not powered state is different than has been experienced elsewhere... I could bad opamp or also different environment (power quality, 50Hz, EMI,...)? Previously I had also issue with oscillating Tridents v1.1...

My best guess is its a bad opamp.
